New Starter/ Employee Changes Form

This form enables us to gather relevant information required to process staff pay and ensure that payslips are distributed correctly and securely through the BrightPay Portal. Please complete the form as thoroughly as possible to ensure that we are able to process payslips efficiently and correctly. Please advise us if there are any changes to your status.

Download the form above and follow the instructions on the right to complete it.
This form authorises Cashtrak to speak to HMRC on your behalf regarding your tax return. You need to complete the following sections:
I (print your name)
Of (business name if applicable)
Complete your address.
Sign and date it. Then on the right hand side, tick the box to the right of Individual*/Partnership*/Trust* Tax Affairs and select Individual from the drop down box.
Enter your National Insurance and UTR.
Send a copy to stacey.woodford@cashtrak.co.uk or Whats App 01865 522785 a photo of the form so we have a signed copy on record.
Print the form off and send it to the address on page 2 of the form.
